Form Display is an application through which end-users access and submit a form. 

Only published form can be displayed in the Form Display. Global styling settings defined in the Live mode will apply.

For web platforms where Smartforms plugins are available.

The main advantage of having Form Display application as a plugin is that it integrates into your website and has full access to environmental variables or website data, such as user information. 


Form Display as plugin will have an admin facility for selecting which form to display.

This admin facility is available only to users that have one of these roles assigned:

 Administrator
 SmartFormsAdmin

Edit Configuration

"Edit Configuration" admin option allows you to select a form that will be displayed in the Form Display. 

Configuring for the first time

When the Form Display application is added to a website page (see installation instructions on how to do it on your platform) no form is yet assigned and the application displays the message "Resource is not configured, please contact administrator".

To select a form to display:

  1. Click on "Edit Configuration" link.

  2. In the "Display Configuration" screen that will open click on "Select a Form" button. 

  3. The drop-down box labelled "Select form" will have all forms in your SmartForms library, incuding the folders they are located in. Choose the form you want to display. Check the form details to make sure you have selected the right form.

  4. If your form is associated only with one live campaign (and that is mostly the case) the section "Campaign Selection" will be pre-filled automatically, no action required. 

    If the form you have selected has more than one campaign, the drop-down box labelled "Select Campaign" will have all live (production) campaigns for the form. Select the campaign you want to collect users data to. For more information on campaigns see Production Campaigns.

  5. Click on "Save Selection" button. 

  6. The Form Display application is now displaying the form you have selected and is ready to accept user entries.

Checking form details

To check the details of the form displayed in the Form Display:

  1. Click on "Edit Configuration" link.

  2. The "Display Configuration" screen will show the details of the form: form name and description, the folder it is located in, etc., as well as the details of the campaign assigned for the Form Display. 

  3. Click "Close Configuration" to return. 

Selecting a different form 

You can change the form you want to display in the Form Display at any time.

To select a different form:

  1. Click on "Edit Configuration" link.

  2. In the "Display Configuration" screen click on the "(Change)" link.

  3. In the drop-down box labelled "Select form" find and select another form. Check the form details to make sure you have selected the right form.

  4. If the form you have selected is associated only with one live campaign (and that mostly the case) the section "Campaign Selection" will be pre-filled automatically, no action required. 

    If the form you have selected has more than one campaign, the drop-down box labelled "Select Campaign" will have all live (production) campaigns for the form. Select the campaign you want to collect users data to. For more information on campaigns see Production Campaigns.

  5. Click "Save Selection" button. The Form Display will display the selected form. 

Selecting a different form for a Form Display application means that users won't have access to the form previously associated with this Form Display application. When they click on the page the Form Display is placed they will see a new form. If they were in the process of filling in the previous form they won't be able to continue. Make sure you give a warning well before switching. 

Selecting a different campaign 

If your form has more than one live (Production) campaign you can change the campaign and start collecting user data into another data set (campaign).

To select a different campaign:

  1. Click on "Edit Configuration" link.

  2. In "Campaign Selection" section of the "Display Configuration" screen click on the "(Change)" link.

  3. In the drop-down box labelled "Select campaign" find and select another campaign. Check the campaign details to make sure you have selected the right one.

  4. Click "Save Selection" button. The Form Test Display will display the same form as before but user entries will be collected into a campaign you have selected. 

Selecting a different campaign for the same form in a Form Display application will not make difference for users that are starting the form. But the users that had started the form before you did the switch won't be able to continue with it, they will be presented with the Start page.
